Apply to Summer Camp-at-Home

Apply to 2021 Summer

Our summer sessions connect campers with similar medical conditions and their siblings and provide community-building experiences through secure online gathering spaces. Campers will enjoy an engaging lineup of real-time camp clubs with special guests, game-themed programs, all-new camp-in-a-box experiences, cabin chats, and more. These virtual programs are offered at no charge.

Programs are open to children who have a diagnosis within the areas associated with one of our core summer programs (see schedule below) or are a brother or sister of a North Star Reach summer camper.

Campers must be 7-15 years old by the first day of the session. Campers for Solstice Camp must be 16 or 17 years old at the start of the session (campers who turn 18 during the session are eligible).

Please note that the registration deadline for the full camp experience, including camp-in-a-box supplies, is June 4 for the first two camp sessions, and June 28 for the remaining camp sessions. This allows time for campers to receive their camp-in-a-box prior to the start of their camp session.

Campers whose applications are received after these deadlines are still welcome to join us, though they may not get camp-in-a-box supplies. Applications may be submitted right up to the day of the session start.

A note for parents about our virtual programs and considerations regarding your child’s  developmental level needs: Camp-at-home programming, much like in-person sessions, hopes to help campers build autonomy, develop competency/mastery of skills and strengthen connections with others. While virtual experiences are certainly different, the outcomes we hope to achieve are interwoven in everything we do.

Our in-person programs are designed for campers who meet the following criteria:

  • Have a minimal developmental age of roughly 7 years old (or no more than 3-4 years delay)
  • Able to communicate independently (with or without assistive devices)
  • Able to function and participate successfully in a group setting (in-person or virtually)

However, the virtual platform has allowed a greater flexibility with eligibility criteria related to camper’s developmental ability and also may require additional support at home depending on the individual. Please complete the registration as accurately and detailed as possible in order for us to have the information needed to understand your camper’s needs. We may follow up with further questions or suggestions on how your child can best enjoy the Camp-at-Home experience.

Apply to Summer Camp-At-Home

Summer Camp-at-Home 2021
Parents and caregivers may register a child or teen for one of our summer “camp-at-home” programs by completing the form below. If you are registering more than one camper, please submit a completed form for each individual child eligible for summer camp.

2021 schedule updates: This summer sibling campers are invited to join one of the condition-specific sessions. Families have the option of having all their campers participate in the same session or selecting a different one to provide siblings with their own special camp-at-home experience. In addition, Solstice Camp will be hosted July 20-23 to align with one of our other sessions. Schedule adjustments were made to allow time for additional registrations and create a larger community for all participants across our core camp sessions.

Summer Camper Parent/Caregiver Consent & Waivers

We ask each parent/guardian to give permission for their child to participate in our camp-at-home sessions by consenting to the following policies, procedures and expected behavior at North Star Reach programs. If you have more than one child registered for a camp-at-home session, you and each child will need to submit a completed form. By clicking on the boxes below, you and your camper acknowledge that you have read and agree to the policies, procedures, media release waivers and code of conduct contract. Required fields indicated by an asterisk *

Program Consent

I realize that no environment is risk-free, and so I have instructed my child on the importance of abiding by the camp’s rules, and my child and I both agree that he/she/they/them is familiar with these rules and will obey them. My child may participate in any and all officially administered, sponsored or sanctioned activities (such as, but not limited to, zoom gatherings, breakout rooms, kids’ kitchen, story studio, sports and games, adventure, nature, and arts & crafts) at North Star Reach. Please understand that we cannot in any way guarantee the security of any outside platform, and you agree that you are responsible for your personal cyber security. *
I understand that because of the virtual camp setting camp will not be providing supervision of my child, and that I am fully responsible for any and all necessary supervision.
I agree that I will not permit any minor who has not been accepted into this program, or whose parents have not signed this document to in any way participate in camp's online activities.
Further, I hereby release, discharge and otherwise indemnify North Star Reach, its affiliated organizations and sponsors, its officers, directors, employees, volunteers and agents (the “Camp Parties”) against any claim by or on behalf of my child as a result of our participation in any program or activity sponsored, coordinated, or supervised by North Star Reach. I also agree to release, discharge and agree to hold harmless and indemnify the Camp Parties with respect to any medical expenses resulting from personal injuries sustained by my child while engaged in such activities.
I attest that my child is age 7-17 years old and has a qualifying medical condition. I understand that acceptance into this year’s programming does not mean automatic acceptance for next year.

Photo / Media Release

I understand that North Star Reach may make audio/visual recordings of the camp-at-home events and activities. I give North Star Reach, SeriousFun Children’s Network, Inc., their respective affiliates and partners (including American Camp Association, University of Michigan Transplant Center, Epilepsy Foundation of Michigan, and its agents) and the parties permitted by them, including media (collectivity, the “Recipients”) permission to have full use of any photographs, slides, moving pictures, and audio/video tapes to promote public understanding and support for children with chronic medical conditions or serious illnesses, public relations and/or advertising.
In the interest of respecting childcare privacy laws, I agree not to take screenshots or to record in any manner of online programming, including zoom gatherings, breakout rooms or comment areas.
If I email North Star Reach photos of my child's art projects and activities during the summer programs, I give my approval for North Star Reach to share those photos in the all-camp photo gallery.

Communication Policy Consent

As part of our policies and procedures, we do not allow program and medical volunteers to communicate with campers and their families via various social media sites, email, mobile phones and mail. Our staff and volunteers participate in an extensive screening, selection and training processes and are supervised by our support team during camp to ensure their behavior is professional and appropriate at all times. We cannot take responsibility for their behavior once camp ended. As the parent/guardian, we ask that you discourage/prevent communication between your camper(s) and camp volunteers outside of official North Star Reach communication channels, and that you immediately report to us any attempts by our staff or your child to engage in any such communication. *
Because we’re unable to gather together in person this year, we’re asking if you’d be willing to allow us to share your camper’s contact information (phone number and mailing address, for mailing purposes) with other campers, allowing kids from the same session the chance to connect outside of camp. During an onsite session, this information is usually shared by campers with their friends on their own.
I understand that if my child has an epilepsy or organ transplant related diagnosis my contact information will be shared with the Epilepsy Foundation of Michigan or the University of Michigan Transplant Center for the purposes of keeping me informed about events and opportunities for my camper and family.

Camp Code Contract

We ask both the camper and the parent/guardian to check the boxes below after you have read the Camp Code Contract. Once you have read the Camp Code Contract, check the box to confirm that you accept the responsibility to meet these behavior expectations.
My camper and I agree to the Camp Code Contract *
